WHAT TO

Expect

First Visit: You may be asked to change into a pair of shorts or t-shirt. Your therapist will do a thorough examination, testing your strength, movement, etc.

If needed, a series of assessments will be completed. This will help narrow down where your issue exists.

After reviewing your medical history, a specific plan of care will be developed and treatment may begin at this time.

Follow up Visits: Typically you will be seen for further treatment after the initial evaluation. The number of treatments will vary and treatments typically last up to one hour.

Insurance: We will process all of your insurance claims whether you are in or out-of-Network.

We are a certified by Ohio BWC, Military plans, Medicare and auto insurance companies.

FREQUENTLY ASKED QUESTIONS

What do I need to bring to my first visit?

Patients need to bring the completed patient paperwork, their insurance card, driver’s license, HSA card if applicable, Doctor’s script or order if applicable. 

How will follow-ups be scheduled?

The staff will put placeholders in our schedule to help accommodate for specific times needed. These placeholders will be reviewed with the patient following the initial evaluation. We will do our best to accommodate schedule needs but dependent on availability. 

How long are appointments?

Initial Evaluations are 1 hour, follow-up visits are approximately 45 minutes but can last up to 1 hour. 

Can I still schedule an evaluation if I'm out of network or don't have any insurance?

For those who have insurance that is not in our network or no insurance  you can come in as a self pay patient. This option usually allows you to get scheduled sooner because we do not need to contact and approve benefits. You would pay a discounted rate at each visit. However, we can give you an itemized bill to turn into your insurance company for possible application to your deductible, co-insurance, etc. HSA cards can be used to pay for  self pay visits.
