Insurance and Medicare
Do You Accept My Insurance?
We are in-network with some insurance plans and out-of-network with others.
Because insurance coverage can vary widely—even within the same provider—we ask that you call our front desk team to confirm whether we are in-network with your specific plan.
Just give us a call—we’ll help you understand exactly what your options are.
What If You’re Not In-Network With My Insurance?
That’s okay! You still have two great options:
1.) If applicable (based on your specific insurance policy) you may use your Out-of-Network Benefits
If we are not in-network with your plan, we can provide you with a detailed receipt (called a “superbill”) that you can submit to your insurance company. This may allow you to be reimbursed, depending on YOUR SPECIFIC out-of-network benefits. We cannot guarantee payment.
2.) Still Get Exceptional Care at a Competitive Value as a cash pay patient.
Even if we’re not in-network, many patients choose us for the quality and value of care they receive. Here’s why:

Do you accept Medicare?
Yes, We Accept Medicare Patients!
At Performance Physical Therapy & Wellness, we proudly welcome Medicare patients. While we are considered a non-participating provider with Medicare, you can absolutely still receive care with us. We’ll explain exactly what that means for you—clearly and simply—so you can feel confident and supported every step of the way.
We are a non-participating provider, which simply means we don’t accept Medicare’s rate as full payment. But you can still receive care with us, and we’ll even submit your claims to Medicare on your behalf.
Q: What does “non-participating provider” mean?
This means we have not signed a contract with Medicare to accept their full reimbursement amount (known as “assignment”). Instead, we collect payment from you at the time of service, and Medicare reimburses you directly.
You may pay slightly more upfront, but many patients find that the care, convenience, and personal attention are worth it.
Q: Will I have to file my own claims?
No—we do that for you!
Our billing team submits your claim to Medicare on your behalf, and you’ll receive a reimbursement directly from Medicare based on your coverage.
Q: Will I pay more out of pocket?
Possibly, but not dramatically.
As a non-participating provider, we are allowed to charge up to 15% more than Medicare’s approved rate. This is called the “limiting charge.” You pay us at the time of service, and Medicare reimburses a portion of that amount directly to you.
We’re happy to give you a clear estimate before your first visit.
