Discovery Visit

This is a 30 minute session where you are scheduled with a Physical Therapist to discuss your situation and determine if therapy is appropriate for you.  If therapy is recommended, an evaluation is typically scheduled.  The fee for this service is $30 and health insurance is not utilized.  

Evaluation – In-network Insurance

This is an hour appointment with a Physical Therapist.  There is generally a 2-3 week wait (in order to get your benefits verified and find an opening).  We send the paperwork to your insurance company and it is processed by them.  Depending on your deductible or co-insurance, there is usually an amount ($125-150) you will be responsible for paying.

Evaluation – Self-pay

This is an hour appointment with a Physical Therapist.  We can typically see you within 24 hours because insurance does not need to be contacted to get approved.  Although, we can give you an itemized bill to turn into your insurance company for possible application to your deductible, co-insurance, etc.  This option is typically utilized if you are in a lot of pain or have a desire to get in quickly to return to activity.  The fee for this service is $150 and is payable at the time of service.

Follow-up visits

Follow-up visits are usually scheduled for 45 minutes.  If follow-up visits are prescribed after the Evaluation, you again have the option to utilize your insurance or you can do self-pay.  Self-pay follow-up visits are usually $75 – $100 each.
